REQUIRED FORMS FOR TRYOUTS
We will be participating in the Greater MKE Power League, which does NOT require a Badger Region Membership. Insurance will be purchased for the PROGRAM and is included in your dues. There will be NO extra cost to families for memberships this season! The only payment due is for the 2024 season which will be $300 per player, accepted with cash or check at tryouts.

Once you are in the form, please see below for information needed about the club to fill it out and submit correctly!
